      Let's simplify a little bit. Eat food, it doesn't matter what, just make sure it's varied. If you don't gain weight, eat more food until you do. Once you have some more meat on your bones you can start tweaking for a specific body composition goal.

      Actually doing something, and adherence to that something, is more important than optimizing things.

      Well, typical recommendations would be NO MORE THAN 2lb MONTHLY. I understand you're underweight, but there's no sense getting fat. I mean, If you're gaining 2lb per month and don;t seem to be gaining to much fat, then go for a slightly higher rate.

      I'm just going to throw out that, after seeing your height and weight, I would more than likely listen to your doctor, not internet opinions.

      Eat your carbs(w/e kinda carbs...) with fats and protein.. and ur good to go... Just be sure to get in enough fiber.
